Return Authorization Process

Complete the required RA Form below for all mail-in equipment repair and calibration service requests. Up to five individual products can be submitted per form (as long as they are to be returned to the same address). Expedited service may be requested for any item, which includes:

• Estimate of repair within 24 hours of receiving your product
• Repair and shipment within 48 hours of receiving the purchase order for items on your estimate

*An additional $250.00 fee per unit will apply. If a technician is unavailable for expedited service, we will notify you following your submission. Once you receive the issued RA #, carefully package your equipment for repair and mail to:

NOTE: If your request involves an item other than a DOD / ChemLogic or New Cosmos branded product, please contact us or await further repair confirmation from a DOD Technologies representative before shipping the product.
